Health Passport is a patient-centered, internet-based health record that allows healthcare providers to overcome the information barriers that exist within a transient population. Using state-of-the-art technology, Health Passport improves care coordination, eliminates redundancy, and reduces provider errors by providing secure access to the member’s treatment history including prescription drug utilization, immunization history, reported allergies, past doctor visits, behavioral health history, laboratory tests, vital signs and patient demographics.

We identified two brothers through Health Passport who were overmedicated and had just transitioned to a new foster home. Our Behavioral Health Service Coordinator assisted the new foster parent in locating a new PCP who began adjusting the medication. With the PCP’s help, the family was connected with a new psychiatrist who agreed with the medication changes. Now, both boys are functioning well with no medication at all.
Two girls – originally adopted internationally were removed from their adoptive home due to abuse and neglect. Until recently, they had refused individual therapy. Through coordination with foster parents and review of Health Passport, we identified a well aligned provider. She has conducted a few sessions with the siblings and now the girls are beginning individual therapy.
