About Oakwood Creative Care (OCC) 

Oakwood Creative Care creates engaging environments and programs that support independence, connection and purpose for older adults while fostering self‑esteem, creativity, lifelong learning and emotional well‑being. Through its Adult Day Club, members participate in meaningful activities tailored to their individual strengths and interests in welcoming, open spaces. 

Oakwood Creative Care also supports caregivers through personalized services offered at The Hub and The Clinic. These services are designed to strengthen caregiving skills, reduce stress, and promote brain health and overall well-being for older adults and their families.

The Centene Foundation Partnership

In 2024, Centene Foundation, in partnership with Arizona Complete Health, committed a multi‑year grant of $2 million to Oakwood Creative Care. Prior to this partnership, Oakwood Creative Care’s Adult Day Club, The Perfect Place, was limited by physical space following the COVID‑19 pandemic, restricting its ability to serve more older adults and caregivers in the community. 

This investment made it possible for Oakwood Creative Care to open a new, purpose‑built campus in Chandler, Arizona, in 2025. The expanded, more accessible location brings the Adult Day Club, Wellness Clinic, and community programming together in one centralized location. 

Programs Supported Through the Partnership 

With support from Centene Foundation, OCC offers a range of programs and services, including: 

  • Adult Day Club services that provide a safe, engaging environment where older adults with cognitive changes can build friendships, express creativity, and thrive. 
  • The Hub, offering monthly social gatherings, workshops and caregiver support groups featuring expert speakers and practical caregiving tools. 
  • A Wellness Clinic, including Oakwood Creative Care’s OakFit Gym with age‑accessible equipment, a relaxation station, counseling services, and additional brain health resources open to the community.

Community Impact

The expansion allows Oakwood Creative Care to serve more individuals, strengthen continuity of care, and better support families navigating dementia and other age‑related challenges. Increased capacity and a welcoming, thoughtfully designed environment broaden access to brain health resources, caregiver support, and services that help individuals age with purpose and community.
