What is LTSS?

Long-term services & supports (LTSS) encompasses a range of services for individuals who have functional limitations due to age, chronic illness, or disability. LTSS services are typically provided on an ongoing and often daily basis, with the goal of supporting individuals' independence, inclusion, and access to needed care and resources.

These essential services assist individuals with daily living activities such as bathing, dressing, and eating, as well as instrumental tasks like grocery shopping, meal preparation, and housework. LTSS uses a whole-person, person-centered approach that promotes dignity and independence by addressing the broader needs of individuals.

Home and Community-Based Services (HCBS)

Long-term services and supports include both home and community-based services, as well as institutional and residential care. HCBS helps individuals live independently in their preferred communities and living arrangements, avoiding institutional care whenever possible.  

 
 

Supporting Members at Home and Beyond

Centene empowers individuals to live independently through programs that include:  

  • Person-centered planning, driven by the member and their chosen circle of support  
  • Integrated care management/service coordination teams  
  • Transition, employment, and housing support
  • Health and wellness programs, including preventative care
  • Assistive technology and home modifications to increase independence and quality of life  
  • Family and caregiver education and support  

 

Person-Centered Thinking

Person-centered thinking is an approach rooted in the belief that people know best about their own lives. It emphasizes listening to and respecting individuals’ preferences, values, and goals. Key principles include respect for choice and control, collaboration, and the use of tools that support meaningful engagement and continuous learning. This approach is embedded in Centene’s core business operations, including LTSS, guiding how services are designed and delivered across the organization.

  • Live safely and independently in their own home and chosen community 
  • Stay connected to family, friends, and community life
  • Maintain their health and wellbeing
