Long Beach, CA
Integrating Health and Housing Solutions in California
Centene Foundation and Health Net are contributing $10M over three years to rehabilitate and expand Encompass Housing's Community Care Campus in Long Beach. Located in a former hospital, the campus provides an integrated service hub for the most vulnerable populations, addressing critical drivers of health such as housing, workforce development, and behavioral health.

Northern Indiana
Addressing the Loneliness Epidemic
Indiana University Health
Indiana University Health Foundation's Congregational Care Network supports seniors and other adults who are experiencing social isolation and need extra support and resources when they are transitioning from hospitals to homes or care facilities. A two-year, $770K commitment from the Centene Foundation and Managed Health Services will help expand the multi-faith network to serve an additional 12 congregations.

Ohio
Strengthening Community-Based Care Coordination
Centene Foundation and Buckeye Health Plan are investing $1.65M over three years to expand the nationally recognized Pathways Community HUB Institute model across Ohio, with a focus on preventing preterm births. The Pathway Community HUBs operate as coordinated care networks that identify high-risk individuals, assess their health and social needs and connect them with appropriate services and resources.

Uvalde, Texas
Uvalde Multi-Purpose Community Health Center Grand Opening
In July 2025, Centene Foundation, Superior HealthPlan, and Community Health Development Inc. (CHDI) celebrated the grand opening of Uvalde's new 16,000-square-foot Multi-Purpose Community Health Center.
First announced in the immediate aftermath of the 2022 Robb Elementary School shooting, the center was made possible through a combined investment of more than $8M from the Foundation and Superior HealthPlan. Providing healthcare services, community outreach, education, and workspaces, the center embodies all three of the Foundation's philanthropic pillars.
